What are "nested hash", "plain hash" and "hash mode"?

I see some comennts in code. It metioned "nested hash", "plain hash" and "hash mode", what do these mean? I only know hash function such as MD5, SHA1, but never heard about the hash mode. Is it similiar with cipher mode, such as CBC,CTR?
