How can i attach a php code to an image?

I hear that there`s a way to attach a code like php code to an image and upload it to a vulnerable site and use it for hacking that. first I just want to know how can we attach an executable code to one file like an image and what that code should contain.and second how that code will execute ??? at last,how can we keep our site safe from attacks like that ???
